REQUIRED MEDICAL FORMS
EMS Dialysis Patient Report
This form must be completed and signed by the patient's treating nephrologist in English, sealed with the hospital stamp, and emailed back to reservation@ems-eg.com at least 2 weeks prior to the patient's arrival to the desired holiday destination.
